My blue couch is brown and dying

I laid blue couch turf in 2003 and did all the right things. Summer of 2004 my lawn kept getting large brown patches and I thought it was lawn grub. I used a lawn grub pesticide. This summer my lawn is now completely brown and patches are dead or dying. Usually blue couch recovers beautifully after rain but mine did not with recent heavy rains. Any suggestions? Thanks
