Detaching from ground?

Hoping someone can help me: It seems like my St. Augustine is detaching from the the groung. It’s only in certain areas. Could I have damaged the roots by raking? I have fertilized, water only two times a week and threw top soil. I haven’t seen any improvement. Any ideas?
